We are primarily engaged in providing skilled care for treatment or rehabilitation services to home bound patients.

Skilled Nursing

When you are recovering from an illness or injury, skilled nursing care works to improve your quality of life by helping you on your road to recovery. Our nurses evaluate your condition on each visit and work with your physician to develop a goal-driven plan of care.

Our skilled nursing care services include but are not limited to:

  • Disease care management (ie. diabetes, cardiac-related diseases, renal failure)
  • Infusion/intravenous therapy
  • Tracheostomy, ostomy, drainage/tube care
  • Medication management
  • Pre-op and post-op care
  • Wound care
  • Pain management
  • Skilled nursing assessment and care coordination
  • Patient/family education on disease process/management
  • Care planning

Occupational Therapy

Occupational therapy focuses on restoring the basic functions of daily living like dressing, grooming, and bathing that have become overwhelming due to a medical condition. It empowers patients to improve their ability to complete Activities of Daily Living (ADLs) and accomplish meaningful activities.

Our occupational therapy services include but are not limited to:

  • Individualized evaluation of patient's ability to perform activities of daily living like bathing, showering, bowel and bladder management, dressing, eating, feeding, functional mobility, personal device care, personal hygiene and grooming
  • Customize a plan of care once abilities and limiting factors are identified to achieve the highest level of independence possible
  • Home safety assessment and recommendation
  • Recommendation for assistive devices, adaptive equipment and training to improve independence in the home
  • Pain assessment and management

Physical Therapy

After an illness, injury or surgery, rehabilitation therapy like physical therapy can make all the difference for a patient. It can improve mobility, aid in recovery, restore physical strength and help a patient gain back independence.

Our physical therapy services include but are not limited to:

  • Functional excercises and treatment to improve ambulation, mobility, independence and circulation
  • Personalized treatment plan to improve mobility, recover from injury and manage pain. As well as prevent future injury to help patients return to living independent lives
  • Patient/family education on posture body mechanics, proper lifting techniques and injury preventions
  • Evaluate patient's progress modifying a treatment plan and trying new treatments as needed.

Speech Therapy

Some types of medical problems like stroke, brain injuries, dementia, hearing loss, or a variety of other medical diagnoses may hinder a patient's ability to communicate needs and desires in an effective manner

Speech therapy assists a patient with swallowing or communication disorders caused by hearing loss, stroke, brain injuries, dementia, or a variety of other medical diagnoses that may cause swallowing difficulties or speech impairment.

Our Speech therapy services include but are not limited to:

  • Assess for any deficits involving voice, communication, and swallowing to allow continued independence and self-expression.
  • Create a treatment plan and various interventions to improve communication, cognition, nutrition, and hydration.
  • Assist patients regain their written, spoken, and non-verbal communication skills as well as their functional independence in swallowing.
  • Assess for dysphagia or difficulty in swallowing to prevent aspiration pneumonia due to impaired swallowing. Teach swallowing techniques and recommend a diet plan that eliminates risk of choking while eating.

Certified Home Health Aide Services

Medical Social Services address the emotional and social repercussion of an illness or injury. A social worker can address the patient's immediate needs and works to remove roadblocks that may stand in the way of recovery. The list of issues that might occur and impede the recovery is endless and includes social, medical, cultural, and psychological problems.

Our social services include but are not limited to:

  • Assessment of patient's needs including psychosocial concerns and how these needs can be met through a collaborative treatment plan
  • Coordination of services such as home health care, medical equipment rentals, transportation to doctor visits, finding funding sources and finding support resources for families
  • Advocate for patient to resolve crises
  • Patient/family referral to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education
